Doc2Form

Doc2Form is an AI-powered converter that turns a PDF, Word document, scanned form, or plain-English description into a native Google Form inside a user's own Google Drive. It is built for teachers, HR teams, recruiters, clinics, agencies and researchers who need to digitize paper forms, quizzes, surveys and intake packets without retyping every question.

A user signs in with Google, uploads a PDF or Word file (or types a description of the form needed), and Gemini AI reads the document to identify questions, answer options, field types and section breaks. The Google Forms API then creates a native Google Form in the user's Drive, which can be edited, shared or connected to Sheets.

Doc2Form maps multiple-choice options to radio buttons, checkbox lists to checkbox fields, short answer prompts to short text fields, rating scales to linear scales, and date fields to date and time pickers, and it reads scanned PDFs with AI vision when plain text tools fail. Uploaded documents are processed to extract structure and then discarded, so only the resulting form title and links are saved to the account.

The first form is free with no credit card required, after which credit packs start at $5 for 10 forms; credits never expire and there is no subscription. Doc2Form is also open source and can be self-hosted for free using Google Apps Script and a personal Gemini API key, with a file size limit of 5 MB per upload.
